Determination of Root Cause
Implementation of Permanent Corrective Action
Systemic Actions to prevent recurrence of the problem

D0 Prepare, Check Emergency Response Action, if required apply ERA 

D1 Establish the Team
Identify champion
Identify team leader
Select members, roles and goals
Establish operating procedure

D2 Description of the problem
 
D3 Interim Containment Action: Define, verify and implement an ICA in order to isolate the effects of the problem from affecting the customer.
 
D4 Define and verify root cause
 
D5 Permanent Corrective Action: Identify possible actions, Select best PCA to remove root cause, Verify actions, verify that no other problems are created.
 
D6 Plan, Implement and validate PCA, Remove ICA, Monitor results
 
D7 Prevent Recurrence, Modify the existing systems to prevent a recurrence of such a problem
 
D8 Recognition, Recognize the efforts and celebrate task completion
